ANÁLISIS DEL IMPACTO DEL TRATAMIENTO PERIODONTAL EN CONTROL METABÓLICO Y CALIDAD DE VIDA EN SUJETOS CON PERIODONTITIS Y DIABETES MELLITUS II QUE CONCURREN A LAS FACULTADES DE ODONTOLOGÍA DE LA UNC Y LA UNCUYO Y FUNDACIÓN INDEPENDENCIA.

ANALYSIS OF THE IMPACT OF PERIODONTAL TREATMENT ON METABOLIC CONTROL AND QUALITY OF LIFE IN SUBJECTS WITH PERIODONTITIS AND DIABETES MELLITUS II WHO ATTEND THE UNC AND UNCUYO FACULTIES OF DENTISTRY AND FUNDACIÓN INDEPENDENCIA.

La periodontitis (EP) es una enfermedad crónica, inflamatoria destructiva que afecta tanto a los tejidos blandos como al hueso que rodea y contiene a los dientes. Datos epidemiológicos recientes informaron que la EP afecta aproximadamente al 47% de los adultos mayores de 30 años y al 64% de los mayores de 65 años. La epidemia mundial de Diabetes Mellitus II (DM) evidencia que es una causa importante de discapacidad y mortalidad prematura. Nos proponemos realizar el diagnóstico y tratamiento de la EP en pacientes con EP y DM, y contribuir a la compensación y seguimiento de las enfermedades crónicas no transmisibles, en un estudio multicéntrico, prospectivo de cohorte en sujetos de dos centros especializados en periodoncia del país. Los profesionales, previamente calibrados, evaluarán el estado de salud del periodonto e indicarán el tratamiento correspondiente con visitas de mantenimiento a los 3 y 6 meses. En cada visita se colectará una muestra de sangre y de saliva para evaluar la concentración de hemoglobina glicosilada, proteína C reactiva y mieloperoxidasa. Cada paciente contestará una encuesta sobre calidad de vida y salud periodontal. Finalmente se realizará un estudio de costo beneficio para conocer si la prevención de las complicaciones periodontales en el sujeto diabético impacta económicamente en el sistema de salud. Esperamos demostrar que el tratamiento periodontal mejora el control metabólico de la glucemia y proponer que en el cuidado de los sujetos diabéticos se incorpore la evaluación de la salud oral de rutina como parte del manejo de la enfermedad. Esta investigación se encuadra dentro del punto 13 (enfermedades orofaciales más prevalentes) de las líneas estratégicas establecidas por las Unidades Académicas de la UNCuyo.

Periodontitis (PD) is a chronic, inflammatory, destructive disease that affects both the soft tissues and the bone that surrounds and contains the teeth. Recent epidemiological data reported that PD affects approximately 47% of adults older than 30 years and 64% of those older than 65 years. The global epidemic of Diabetes Mellitus II (DM) shows that it is an important cause of disability and premature mortality. We intend to carry out the diagnosis and treatment of PD in patients with PD and DM, and contribute to the compensation and follow-up of chronic noncommunicable diseases, in a multicenter, prospective cohort study in subjects from two centers specialized in periodontics in the country. The professionals, previously calibrated, will evaluate the health status of the periodontium and will indicate the corresponding treatment with maintenance visits at 3 and 6 months. At each visit, a blood and saliva sample will be collected to evaluate the concentration of glycosylated hemoglobin, C-reactive protein, and myeloperoxidase. Each patient will answer a survey on quality of life and periodontal health. Finally, a cost-benefit study will be carried out to find out if the prevention of periodontal complications in diabetic subjects has an economic impact on the health system. We hope to demonstrate that periodontal treatment improves metabolic glycemic control and propose that routine oral health assessment be incorporated into the care of diabetic subjects as part of disease management. This research falls within point 13 (most prevalent orofacial diseases) of the strategic lines established by the Academic Units of the UNCuyo.

Cellular and molecular study of autophagy during human erythropoiesis.

Alteraciones en un proceso normal de diferenciación y maduración, como es la hematopoyesis, pueden conducir a situaciones de sobreproducción de células hematopoyéticas (como las leucemias), o a una producción deficiente de las mismas (anemia aplásica). A través de la autofagia, macromoléculas citosólicas e incluso organelas enteras son degradadas en lisosomas. Durante la eritropoyesis los precursores de los glóbulos rojos sufren una extensiva remodelación de sus membranas, disminución del volumen celular, producción de proteínas específicas (hemoglobina) y eliminación de organelas membranosas (mitocondrias y ribosomas) por autofagia. En este proyecto, postulamos que la autofagia posee un rol crucial en la maduración de las células eritroides, por lo que el objetivo general consiste en establecer los mecanismos celulares y moleculares de la vía autofágica mediada por un receptor de membrana (LRP1), que participan durante la eritropoyesis. Este receptor, (proteína 1 relacionada con el receptor de lipoproteína de baja densidad, también llamado CD91) está involucrado en muchos procesos celulares tales como el metabolísmo de la alfa-2-macroglobulina y de la apolipoproteína E, migración celular, apoptosis y autofagia. Por otro lado, el estudio de los mecanismos de exocitosis, la composición y cantidad de exosomas liberados durante la eritropoyesis otorgaría datos importantes de la posible comunicación intercelular durante dicho proceso. Es por ello, que el estudio en profundidad de los mecanismos moleculares y celulares que regulan la autofagia y la actividad del receptor LRP1, otorgará un valioso conocimiento para el entendimiento, no solo de la fisiología del proceso de maduración eritroide, sino también del tratamiento de aquellas hematopatologías asociadas a este proceso, como las leucemias.

Alterations in a normal process of differentiation and maturation, such as hematopoiesis, can lead to situations of overproduction of hematopoietic cells (such as leukemia), or their deficient production (aplastic anemia). Through autophagy, cytosolic macromolecules and even entire organelles are degraded in lysosomes. During erythropoiesis, the precursors of red blood cells undergo extensive remodeling of their membranes, decrease in cell volume, production of specific proteins (hemoglobin), and elimination of membranous organelles (mitochondria and ribosomes) by autophagy. In this project, we postulate that autophagy plays a crucial role in the maturation of erythroid cells, so the general objective is to establish the cellular and molecular mechanisms of the autophagic pathway mediated by a membrane receptor (LRP1), which participate during erythropoiesis. This receptor, (low-density lipoprotein receptor-related protein 1, also called CD91) is involved in many cellular processes such as alpha-2-macroglobulin and apolipoprotein E metabolism, cell migration, apoptosis, and autophagy. On the other hand, the study of the mechanisms of exocytosis, the composition and quantity of exosomes released during erythropoiesis would provide important data on the possible intercellular communication during said process. For this reason, the in-depth study of the molecular and cellular mechanisms that regulate autophagy and the activity of the LRP1 receptor will provide valuable knowledge for the understanding, not only of the physiology of the erythroid maturation process, but also of the treatment of those hematopathologies associated with this process, such as leukemia.

Los biopolímeros sintéticos, como el PLA (Ácido poliláctico), o el PLA plus (sometido a un proceso de recocido previo a su fundido para disminuir su higroscopía), por ser reabsorbibles, vienen siendo estudiados ampliamente desde varios puntos de vista, pensados como posible carrier o coadyuvantes de injerto de células madres para la regeneración tisular, como vehículo de factores estimuladores para el mismo fin, o bien para la administración local de medicamentos.Sus propiedades mecánicas, tales como la resistencia compresiva, traccional o flexural y la notoria higroscopía que presentan, son bien conocidas. Sin embargo, cómo se verán alteradas en el tiempo, tras ser sometidas a condiciones similares a las de su injerto, no están tan claras. Este trabajo busca analizar el comportamiento del PLA y el PLA plus, utilizando diversas configuraciones de impresión, en el aspecto de la resistencia compresiva, la estabilidad dimensional y/o volumétrica, tras ser sometida a condiciones simil injerto, in vitro. Adicionalmente, habiendo también resultados diversos del estudio del efecto de la esterilización por autoclave de dicho material en los aspectos mencionados, se analizara esta variable también.Teniendo en claro su comportamiento, las técnicas de regeneración ósea y tisular en general, combinadas con el injerto en bloque de estos biopolímeros, serían mucho más predecibles en sus resultados.

Synthetic biopolymers, such as PLA (polylactic acid), or PLA plus (subjected to an annealing process prior to melting to reduce its hygroscopicity), because they are resorbable, have been extensively studied from various points of view, thought as possible carrier or adjuvants for stem cell grafting for tissue regeneration, as a vehicle for stimulating factors for the same purpose, or for the local administration of medications.Their mechanical properties, such as compressive, tractional or flexural strength and their remarkable hygroscopicity, are well known. However, how they will be altered over time, after being subjected to conditions similar to those of their graft, is not so clear.This work seeks to analyze the behavior of PLA and PLA plus, using various printing configurations, in terms of compressive strength, dimensional and/or volumetric stability, after being subjected to graft-like conditions, in vitro.Additionally, having also diverse results of the study of the effect of the autoclave sterilization of said material in the mentioned aspects, this variable will also be analyzed.With their behavior clear, bone and tissue regeneration techniques in general, combined with block grafting of these biopolymers, would be much more predictable in their results.

Trypanosoma cruzi es el agente causal de la enfermedad de Chagas y se estima que actualmente existen en el mundo más de 8 millones de personas infectadas con este parásito. T. cruzi en su forma infectiva, conocida como tripomastigote, es capaz de infectar una gran diversidad de tipos celulares en mamíferos. Cuando el tripomastigote ingresa a la célula hospedadora, forma un compartimento conocido como vacuola parasitófora de T. cruzi (TcPV). La TcPV debe fusionarse posteriormente con lisosomas que aportan las condiciones necesarias para que el ciclo de T. cruzi continúe en el interior de la célula hospedadora. En resultados previos encontramos que las proteínas Rab9 y Rab39, son reclutadas a la membrana de la vacuola parasitófora. En este trabajo nos proponemos estudiar el rol de ambas Rab en la maduración de la TcPV y sus posteriores efectos en el desarrollo de la infección.

En epidemiología existen estudios y descripciones en la salud y enfermedades que ocurren en una población determinada, teniendo en cuenta una serie de Patrones de la enfermedad: el tiempo en el que aparece, que es más frecuente, el lugar en donde aparece la enfermedad, y aquellas personas con más probabilidades de sufrirla. Estas enfermedades en la endodoncia son las patologías pulpares y periapicales. Un estudio retrospectivo lineal se llevará a cabo en pacientes tratados durante el período comprendido entre Junio de 2013 a Junio de 2016 en la especialidad de Endodoncia Facultad de Odontología UN Cuyo. Para el estudio epidemiológico se tomarán en cuenta las siguientes variables: edad, sexo, dirección de paciente, diente afectado, patología presente. El objetivo de este estudio será realizar un estudio epidemiológico de los datos recolectados de la historia clínica diseñada para la atención de los pacientes tratados en la Carrera de especialización en endodoncia. In epidemiology there are studies and descriptions of health and diseases that occur in a given population, taking into account a series of patterns of the disease: the time in which it appears, which is more frequent, the place where the disease appears , and those people most likely to suffer from it. These diseases in endodontics are pulpal and periapical pathologies. A linear retrospective study will be carried out in patients treated during the period from June 2013 to June 2016 in the specialty of Endodontics, Faculty of Dentistry UN Cuyo. For the epidemiological study, the following variables will be taken into account: age, sex, patient's address, affected tooth, present pathology. The objective of this study will be to carry out an epidemiological study of the data collected from the clinical history designed for the care of patients treated in the Endodontics Specialization Course. Efectos de la senescencia sobre la actividad fogocítica

Senescence effects in the the phagocytic pathway

El funcionamiento en el organismo de diferentes tejidos y sistemas sufre modificaciones debido al envejecimiento. Esto se debe al hecho de que las células acumulan daños tanto a nivel molecular en proteínas, lípidos y material genético, como a nivel de orgánulos. Para contrarrestar el efecto de esta acumulación y de sus efectos dañinos, las células tienen diferentes mecanismos, como la autofagia, la senescencia o la apoptosis. La autofagia es la primera línea de defensa, previniendo la acumulación de moléculas y organelas dañadas. Cuando la autofagia no es suficiente, se activan otros mecanismos que evitan que esas células dañadas se propaguen, como la apoptosis donde las células mueren, o la senescencia donde se frena la replicación celular. Particularmente, el sistema inmune no escapa a este deterioro y se somete a un proceso llamado inmunosenescencia, que se caracteriza por un estado inflamatorio crónico, pero que al mismo tiempo presenta una menor capacidad de reacción a los antígenos. Concomitantemente a este cambio en el sistema inmune se observa una disminución en la capacidad de enfrentar a patógenos y una mayor predisposición a procesos inflamatorios crónicos. Un ejemplo de esto es la mayor incidencia de la periodontitis crónica en personas ancianas. La periodontitis, una de las enfermedades con más prevalencia en el mundo, es un cuadro inflamatorio crónico de etiología bacteriana que lleva a la destrucción del tejido periodontal. El objetivo de este proyecto es analizar los cambios en la capacidad fagocìtica de macrófagos y neutrófílos debidos a la activacion de la senescencia, y estudiar si existe una relación entre la activación de la senescencia de neutrófilos y la progresión de la periodontitis.Además, encontrar un agente capaz de prevenir o frenar estos procesos constituye una interesante opción terapéutica. La melatonina (MEL), hormona secretada por la glándula pineal, además de su efecto sincronizador sobre los ritmos circadianos, posee diversas propiedades que están en investigación. Se sabe que la melatonina tiene efectos antinflamatorios y promueve además la formación ósea, por lo que sería un interesante candidato para el tratamiento de la enfermedad periodontal. Es por ello que además en este proyecto estudiaremos in vitro si la MEL puede revertir los efectos de la activación de la senescencia sobre la vía endofagocítica.

The functioning in the body of different tissues and systems undergoes modifications due to aging. This is due to the fact that cells accumulate damage at the molecular level in proteins, lipids and genetic material, as well as at the level of organelles. To counteract the effect of this accumulation and its harmful effects, cells have different mechanisms, such as autophagy, senescence or apoptosis. Autophagy is the first line of defense, preventing the accumulation of damaged molecules and organelles. When autophagy is overpass, other mechanisms are activated to prevent damaged spreading, such as apoptosis where cells die, or senescence where cell replication is stoped. Particularly, the immune system undergoes a process called immunosenescence, which is characterized by a chronic inflammatory state, but which has a lower ability to react to new antigens. Concomitant to this change in the immune system is a decrease in the ability degraded some pathogens and a greater predisposition to chronic inflammatory processes. An example of this is the higher incidence of chronic periodontitis in elderly people. Periodontis, one of the most prevalent diseases in the world, is a chronic inflammatory disease of bacterial etiology that leads to the destruction of periodontal tissue. The objective of this project is to analyze the changes in the phagocytic capacity of several cell lines due to the activation of senescence, and to study if there is a relationship between the activation of neutrophil senescence and the progression of periodontitisIn addition, finding an agent capable of preventing these processes constitutes an interesting therapeutic option. Melatonin (MEL), a hormone secreted by the pineal gland, in addition to its synchronizing effect on circadian rhythms, has various properties that are under investigation. Melatonin have an anti-inflammatory effects and promotes bone formation, making it an interesting candidate for the treatment of periodontal disease. For this reason, in this project we will also study in vitro whether MEL can reverse the effects of senescence activation on the endophagocytic pathway.

El presente proyecto desarrollará un estudio observacional transversal correlacional con el objetivo de poner de manifiesto la relación entre los defectos del desarrollo del esmalte (DDE) y la Caries de la Infancia Temprana (CIT) en niños entre 24 y 60 meses de edad con desnutrición infantil aguda y crónica que concurren a Centros de Prevención y de Atención de la Fundación CONIN en Mendoza. Los DDE ocurren cuando algún evento desfavorable altera el proceso de amelogénesis. Entre los múltiples factores que intervienen, la desnutrición infantil temprana ha sido descripta como posible factor causal. En una primera etapa, en un estudio anterior de éste equipo de investigación, se logró establecer que los DDE afectaron a la dentición temporaria de los niños con desnutrición de Mendoza, siendo la forma difusa la más frecuente y abarcando varios elementos dentarios. En esta segunda etapa partiendo del hecho que DDE constituyen un factor de riesgo adicional para CIT, se pretenderá como objetivo el establecer la posible relación entre DDE y CIT en niños con desnutrición crónica y aguda en niños de Mendoza estudiados. Se utilizará el Índice de Defectos del Desarrollo Modificado para cuantificar el problema, determinándose la prevalencia de DDE por unidad boca y la prevalencia de cada uno de los DDE, así como la distribución por elemento dentario y cantidad de elementos afectados por niño, y la prevalencia y severidad de caries midiendo el estado dental a través de índices ceod y ceos que incluirán el registro de lesiones de esmalte y dentina según el sistema de detección de caries activas ICDAS II. Se realizaran las pruebas de asociación correspondientes para hallar posibles relaciones entre DDE y CIT en los niños con desnutrición infantil que compondrán la muestra.Los DDE y la CIT en niños con desnutrición infantil presentan relaciones de riesgo que deben ser esclarecidas en orden de objetivar el impacto de la primera variable sobre la segunda en los niños de Mendoza.

This project will develop a correlational cross-sectional observational study with the aim of revealing the relationship between developmental enamel defects (EDD) and Early Childhood Caries (ECC) in children between 24 and 60 months of age with child malnutrition, acute and chronic, who attend the Prevention and Care Centers of the CONIN Foundation in Mendoza.EDDs occur when some unfavorable event alters the amelogenesis process. Among the many factors involved, early childhood malnutrition has been described as a possible causal factor. In a first stage, in a previous study by this research team, it was possible to establish that EDDs affected the primary dentition of children with malnutrition in Mendoza. The diffuse form was the most frequent and encompassing various dental elements. In this second stage, based on the fact that EDD constitutes an additional risk factor for ECC, the objective will be to establish the possible relationship between these variables in children with chronic and acute malnutrition from Mendoza.The Modified Developmental Defects Index will be used to quantify the problem, determining the prevalence of EDD per mouth unit and the prevalence of each of the EDD, as well as the distribution by dental element and number of elements affected per child, and the prevalence and severity of caries by measuring dental status through ceod and ceos indices that will include the registration of enamel and dentin lesions according to the ICDAS II active caries detection system. The corresponding association tests will be carried out to find possible relationships between DDE and CIT in the children with childhood malnutrition that will make up the sample.The DDE and the CIT in children with child malnutrition present risk relationships that must be clarified in order to objectify the impact of the first variable on the second in the children of Mendoza.

Se pretende determinar los niveles de daño oxidativo en el ADN en los pacientes con leucoplasia, liquen oral y queilitis actínica, su relación con la expresión de &H2AX, compararla con zonas sanas, sin lesiones bucales del mismo paciente y con los valores obtenidos a lo largo del curso evolutivo de las patologías después de 1 mes, 3 meses y 6 meses de tratamiento. Se proyecta determinar las variaciones en dichos niveles que ocurren con el tratamiento con imiquimod, corticoides locales o tretinoína local en estos pacientes. Se determinarán la presencia de aductos oxidativos del ADN a través de la titulación en células exfoliadas de la mucosa oral con anticuerpo anti 8OH guanosina (8OH-G). Además se plantea realizar la determinación de &H2AX, en las diferentes etapas de los tratamientos ya que en un estudio previo hemos observado diferencia significativa entre el nivel de estrés oxidativo en las primeras etapas del tratamiento y su descenso con posterioridad. Se ha demostrado que el incremento del estrés oxidativo incrementa la expresión de &H2AX, que es una histona fosforilada que aparece cuando hay roturas en la cadena de ADN.

The aim is to determine the presence of oxidative DNA adducts (8OH- guanosine) in patients with leukoplakia, oral lichen and actinic cheilitis, their relationship with the expression of &H2AX, compare it with healthy areas, without oral lesions of the same patient and with the values obtained throughout the evolutionary course of the pathologies after 1 month, 3 months and 6 months of treatment. It is planned to determine the variations in these levels that occur with treatment with imiquimod, local corticosteroids or local tretinoin in these patients. The presence of oxidative DNA adducts will be determined in exfoliated mucosal cells with anti-8OH-guanosine antibody (8OH-G). In addition, it is proposed to determine the expression of &H2AX, in the different stages of the treatments, since in a previous study we have observed a significant difference between the level of oxidative stress in the early stages of treatment and its decrease subsequently. It has been shown that increased oxidative stress increases the expression of &H2AX, which is a phosphorylated histone that appears when there are breaks in the DNA chain.

En los proyectos desarrollados, observamos la importancia que adquiere en la zona de estudio, la figura del estado. Esta propuesta se enfoca en la información pública y la comunicación, sobre diversas problemáticas, como el acceso a los censos de población, a las estadísticas de enfermedades, para poder contribuir con las posibles soluciones que sean acordes a los desarrollos muy propios, respetando las costumbres y cultura del lugar.El objetivo es relevar información pública y no pública y su acceso para las comunidades, relacionadas con la salud y bienestar de la población, para que ellas puedan adecuar su planificación. Detectar enfermedades bucales y de origen zoonóticas.La metodología consiste en la búsqueda y recolección de información a través de entrevistas a informantes clave, a diferentes actores institucionales y de las comunidades, con una perspectiva histórica de la salud y la enfermedad en el contexto del secano. Se realizarán campañas de inspección en escuelas y centros de salud, además de anamnesis a la población que acuda a las campañas, para contribuir a la prevención.Uno de los problemas claves que encontró el equipo en el desarrollo de la investigación es el del agua, eje central para la vida.En este lugar se encontró cantidad de arsénico en el agua, en algunos casos excesivo. Para paliar parte del problema, el Estado construyó un acueducto de 280 km cuyo origen es un pozo de Gustavo André. Pero el agua no es totalmente libre de arsénico. Luego de un trabajo conjunto, se decidió repartir en los puestos un filtro creación de un investigador de la Universidad Nacional de San Juan, para que los pobladores filtren el agua que van a consumir. Sin embargo, los filtros que se repartieron como prueba se saturan en pocos meses y su costo es demasiado elevado. Además, no se ha encontrado una solución sobre un lugar seguro para acumular el arsénico. Por otra parte, hay zonas donde ya no llega el agua, la del acueducto. Es necesario buscar otro pozo, libre de arsénico.Por otra parte, la población no accede a los datos públicos a los que tiene derecho. No ha obtenido respuestas a sus reclamos de datos censales por Comunidad, y otros estudios a los que han aportado información, pero no se les brinda los resultados de éstos. Se espera obtener información pública y no pública, mejorar la comunicación a la población interesada, obtener conocimiento sobre enfermedades bucales y de origen zoonóticas para aportar a la prevención.

In the developed projects, we observe the importance that the figure of the state acquires in the study area. This proposal focuses on public information and communication, on various problems, such as access to population censuses, disease statistics, in order to contribute with possible solutions that are consistent with the very own developments, respecting the customs and culture of the place.The objective is to reveal public and non-public information and its access for the communities, related to the health and well-being of the population, so that they can adapt their planning. Detect oral and zoonotic diseases.The methodology consists of the search and collection of information through interviews with key informants, with different institutional and community actors, with a historical perspective of health and disease in the dryland context. Inspection campaigns will be carried out in schools and health centers, in addition to anamnesis of the population that attends the campaigns, to contribute to prevention.One of the key problems that the team found in the development of the research is that of water, the central axis for life.In this place, an amount of arsenic was found in the water, in some cases excessive.To alleviate part of the problem, the State built a 280 km aqueduct whose origin is a Gustavo André well. But the water is not totally free of arsenic. After joint work, it was decided to distribute a filter created by a researcher from the National University of San Juan, so that the residents can filter the water they are going to consume. However, the filters that were distributed as a trial are saturated in a few months and their cost is too high. Also, no solution has been found on a safe place to accumulate arsenic.
